Wounds heal best when kept dry and exposed to the air so that a hard scab can form.

What we know now

Many wounds heal faster in a protected, appropriately moist environment. Moisture supports cell migration and tissue repair, while excessive dryness can slow healing.

Why it changed

Traditional wound care aimed to dry wounds using air exposure, heat lamps, iodine, and absorbent dressings. Experiments beginning in the 1960s showed faster epithelial growth beneath moisture-retaining coverings.

A dry scab looks protective. It forms a visible barrier, reduces surface moisture, and can make a wound appear sealed off from contamination. Traditional wound care therefore often aimed to dry wounds aggressively.

Experimental work beginning in the 1960s showed that this visible crust was not necessarily ideal for tissue repair.

Epithelial cells migrate across the wound surface more efficiently when the environment remains appropriately moist. Under excessively dry conditions, a hard scab can become an obstacle that cells must move beneath while the exposed tissue dehydrates.

Modern dressings therefore often aim to protect the wound while maintaining controlled moisture.

That does not mean “the wetter, the better.” Excess fluid can macerate surrounding skin, worsen leakage, and create other problems. Infection, drainage, wound depth, blood supply, and underlying disease also change what treatment is appropriate.

The superseded rule is the universal dry-and-open ideal. For many wounds, protected moist healing produces faster and better epithelial repair than deliberate desiccation.
