Asbestos dust may irritate the lungs, but it does not cause cancer.

What we know now

Inhaled asbestos fibers cause mesothelioma and increase the risks of lung, laryngeal, and ovarian cancers. They also cause asbestosis and other pleural disease.

Why it changed

Early occupational reports focused on fibrosis and often treated cancer cases as incidental. Cohort studies of heavily exposed workers and the distinctive association between asbestos and mesothelioma established a causal relationship.

Early occupational medicine recognized that heavy asbestos exposure could scar the lungs, producing asbestosis. Cancer was harder to connect.

One reason was latency. Mesothelioma and lung cancer often appear decades after the exposure that caused them, long after workers had changed jobs or retired.

As larger occupational cohorts were followed, the pattern became unmistakable. Workers with heavy asbestos exposure developed excess cancers, and the striking association with mesothelioma provided an especially distinctive signal.

The evidence eventually established asbestos as a human carcinogen as well as a cause of fibrotic lung disease.

Risk depends on dose, fiber characteristics, duration of exposure, and other factors. Smoking interacts strongly with asbestos exposure in causing lung cancer.

That interaction does not make asbestos harmless, and smoking does not explain the asbestos–mesothelioma relationship.

The historical delay illustrates why long-latency occupational hazards can remain underestimated even after more immediate tissue damage is already recognized.
